"In part because of my own experience of living successfully with chronic illness, I have worked skillfully and compassionately with those challenged by acute or chronic illness, invisible or visible, and with those close to them. Whether you have an illness or are close to someone with an illness, illness is always stressful. Illness can also open the way to healing beyond the physical, leading to deeper and closer relationships and fuller understanding of ourselves."

"I am a local Clinical Psychotherapist in the Reston/Herndon who specializes in treating all sorts of chronic illness, including fibromyalgia/CFS.  I have been working with on of your members and others like her to help support these patients and their families through the oft resulting significant mental health sequelea of dealing with this difficult illness."
Recommendation by neurologist, psychiatrist, Reuben Cintron, MD, "Ms. McKay is a therapist in Herndon, VA, who has been instrumental in helping many of my patients assimilate to the psychological consequences of chronic illnesses specifically head injury with postconcussion syndrome, migraines, and other chronic neurological illnesses in my practice.  I have been working for a number of years looking for the perfect fit for my patient's chronic illnesses and I think I finally found it.  She is compassionate, empathetic and very effective at helping patients learn coping strategies.  She has been very good at communicating with me and I look forward to many years of our continued relationship.  She has assimilated very well to a wide range of cases with patients of different educational background, cognitive performance, and illnesses.  I highly recommend her to work with any of your patients and I have told Ms. McKay that I would be honored to serve as a reference source."

"I am a speech-language pathologist and have spent the last 25 or so years working with people who have a variety of communication, learning and cognitive challenges. My specialty is the use of technology along with a holistic 'life participation' approach. I offer short-term intensive therapy as well as distance coaching and training.  I have worked with a few of internist Dr. Pocinki's patients who seem to benefit from my approach." - Joan Green
Author -
Technology for Communication and Cognitive Treatment: The Clinician's Guide
Dr Pocinki's comment, "I found a a speech therapist by training, who was equally frustrated that speech therapists working with cognitively impaired patinets were using techniques that were 30 years old.  Joan Green just spent 4 years compiling various electronic resources, published  in June 2007.  Though much of it is geared toward kids with learning disabilities and adults with neurological diseases, there are some things that are applicable to CFS patients.  She was interested in learning about CFS, and has seen a few of my patients, mostly for one-time consults to help develop a plan for them and focus their recovery efforts.   Of course, the cognitive problems are one of the hardest things to treat, so just having some options is a big plus."

NEURO-PSYCH TESTING

Jump down to Practitioners

Articles about Cognitive/Neuropsychological Testing:
  • Cognitive Function in CFS Eleanor Stein, MD, FRCP(C)

Tests which most commonly discriminate CFS from controls:

Any test measuring functioning before and after exertion
Psychomotor speed - Finger Tapping Test (Halstead 1947),
slower and decreasing speed across trials
Memory - Selective Reminding Task (Rey 1964) delayed recall and long term storage
Auditory processing speed - PASAT (Stuss et al 1987)
Attention - Digit span forwards and backwards (Wechsler 1955),
Trail making test parts A&B (Reitan & Davidson 1974)

Conclusions: As a group persons with CFS have measurable cognitive impairment which is made worse by physical exertion and cannot be attributed to comorbid conditions or attitudes. In individual patients cognitive deficits are difficult to prove with neuropsychiatric testing since there are rarely premorbid test results with which to compare. Never the less, findings which are out of keeping with known levels of premorbid functioning and decreased functioning after aerobic exercise are consistent with a diagnosis of CFS. Healthy controls and persons with depression generally do better on neuropsychiatric tests after exercise.

  • The Role of Neuropsychology in CFIDS - Leo J Shea III PhD This article explores the surprising benefits of a neuropsychological evaluation and guides you through the evaluation process.
  • Is CFS a Brain Disorder? - Lange / The Science and Research of CFS 2005-2006
The cognitive problems CFS patients report have led to numerous research studies to assess neurocognitive impairments.  Although data is not consistent, research confirms both structural
and functional brain abnormalities in CFS.
  • From Protocol for Cognitive Assessment for CFIDSSandman PhD & Moore PsyD
The unique and specific effects of CFIDS included pathological overestimation of ability (metamemory), inability to recall information even with cues, serious distractibility and inability to concentrate, and slowed mental processing and decision-making. The protocol for these deficits involves three tests. The most sensitive is the PI test. The recall test adds to the sensitivity of the profile. The item recognition test produces a modest improvement on the utility of the specific battery to detect CFIDS deficits.

  • Cognitive Function & Fibromyalgia  Jennifer Glass PhD & Denise Park PhD

"I can deal with the pain, but the memory and thinking problems really worry me."
"The physical symptoms weren't as frightening as the memory problems."
"The most devastating effect for me has been the cognitive impairment."

If you have fibromyalgia (FM), then these quotes may describe what you have felt as well. Many FM patients complain of cognitive (or mental) symptoms such as memory failures (both long-term and short-term), difficulties with attention, and with finding the right words. Our research has shown that patients' complaints about their cognitive problems are accurate: there is cognitive dysfunction in FM patients and this is not due solely to psychiatric disorders such as depression, or to other symptoms of FM such as sleep disturbance, anxiety or fatigue. On the other hand, cognitive dysfunction in FM is related to pain.
